Professionally complete case management functions that include but are not limited to intake, assessments, treatment planning, on-going individual sessions to monitor progress and facilitate problem-solving, educating clients regarding resources, and consulting/coordinating with other members of the integrated team and community resources. Provide regular training sessions to clients, regarding information on employment readiness, resumes, interviewing, networking, housing opportunities, home care techniques, tenant responsibilities, and budgeting.
