Coordinates discharge planning services for assigned areas to include screening for high-risk factors, facilitation and coordination of transfer/referral of patients' care to other facilities, community or home-based services.
The selected individual will have primary responsibilities for serving as a supervising/attending faculty for the GPR-P residents, as well as assist the Program Director with administration duties, including but not limited to: (a) development and implementation of the curriculum; (b) evaluation of program content; (c) oversight and evaluation of resident and faculty at off-site rotations; (d) maintenance of residents’ education records; and (e) application, interview, recruitment, and selection of residents.
