Demonstrated knowledge AND at least six (6) years of work experience OR one of the following education requirements (NOTE: degree not required if work experience meets the same level of education): Associate's degree in human services, social science or health education PLUS four (4) years of case management experience, or. Complete intake into care, including comprehensive initial and ongoing assessments of patient strengths and needs, including HIV medical status, insurance coverage, financial status, adherence to appointments and medications, behavioral health and substance use, oral health, nutrition, transportation, and other psychosocial needs.
