Identify and pursue new referral sources, including hospitals, skilled nursing and rehab facilities, assisted living communities, physician practices, case managers, social workers, and community organizations. • Comfortable with field-based work and travel throughout the territory, which may include Allegheny, Armstrong, Beaver, Berks, Bucks, Chester, Delaware, Lancaster, Lehigh, Montgomery, Northampton, Philadelphia, Washington, and Westmoreland counties.
