Cooperidge Consulting Firm is seeking a Physical Therapist for a premier health organization in Elmira, NY.This is a high-impact, Direct Hire opportunity for a compassionate therapist to join a top-tier rehabilitation team. While the role is based within an Acute Care setting, it offers the patient-centered focus of a specialized clinic. We are looking for a motivated professional (Entry Level to Experienced) to provide comprehensive rehabilitation care to a diverse patient population. Job Responsibilities<ul><li>Clinical Evaluation: Conduct comprehensive initial evaluations and develop individualized, evidence-based treatment plans tailored to specific patient needs.</li><li>Therapeutic Intervention: Administer a wide range of treatments, including manual therapy, therapeutic exercises, and modalities (Ultrasound, E-Stim, Heat/Ice therapy).</li><li>Weekend Coverage: Provide critical rehabilitation services during weekend shifts, ensuring continuity of care and optimal patient recovery timelines.</li><li>Patient & Family Education: Empower individuals by educating them on posture correction, injury prevention, and customized home exercise programs.</li><li>Multidisciplinary Collaboration: Partner closely with physicians, nurses, and other specialists to provide holistic, patient-centered care within an acute setting.</li><li>Outcome Monitoring: Diligently monitor patient progress and adjust treatment protocols in real-time to meet functional rehabilitation goals.</li><li>Clinical Documentation: Maintain accurate and timely electronic documentation, including progress notes and discharge summaries, in compliance with industry standards.</li><li>Safety & Compliance: Adhere to strict infection control, safety guidelines, and departmental policies to maintain a safe healing environment.</li></ul>Requirements<h3>Education & Licensing</h3><ul><li>Doctor of Physical Therapy (DPT) or equivalent degree from an accredited program is REQUIRED.</li><li>New York State PT License (or immediate eligibility to obtain) is REQUIRED.</li><li>Basic Life Support (BLS) certification required within 90 days of hire.</li></ul><h3>Experience & Skills</h3><ul><li>Entry Level candidates are welcome; prior experience in outpatient or acute care settings is preferred.</li><li>Ability to deliver compassionate care for orthopedic, neurological, and musculoskeletal conditions.</li><li>Strong organizational and time-management skills with a focus on collaborative communication.</li></ul><h3>Physical Requirements</h3><ul><li>Ability to stand and move for extended periods.</li><li>Must be able to lift up to 30 lbs independently and up to 150 lbs with assista</li></ul>BenefitsWork Model: 100% On-site (Elmira, NY).Sign-On Bonus:$15,000.00 to welcome you to the team.Weekend Pay:25% Premium on top of your base rate for all hours worked on Saturday and Sunday.Compensation: Competitive salary ($70k–$100k) plus additional pay for relevant certifications.Benefits: Comprehensive health, dental, vision, and retirement package.Relocation:Full relocation assistance available.Development: Continuous education opportunities, clinical mentorship, and professional workshops.

Position Summary: Responsible to collect and record specific data elements from patient medical records based on established criteria and guidelines. (This data will be used in an aggregate database for performance improvement processes). Performs data entry for clinical data systems.Education, License & Cert: High School graduate or GEDExperience: Minimum one‐year experience working with medical records or results reporting (i.e., one‐year unit clerk), Anatomy and Physiology or related course work, and Basic Medical Terminology course. Computer/typing/data entry skills desired. Prior exposure to cardiovascular terminology preferable. Or Equivalent of one‐year proven experience with medical terminology and procedures (i.e., auditing, coding, abstracting, work in a physician's office and will complete the Basic Medical Terminology Course if selected for the position. Computer/typing/data entry skills desired.Essential Functions:<ol><li>Abstract clinical findings from designated medical records and/or electronic medical record systems for use in internal performance improvement as well as for defined regulatory requirements</li></ol>a. Utilizes established guidelines, data definitions, and criteria.b. Retrains to guideline revisions and updates as needed.c. Performs data entry according to specified format.d. Generates established reports as needed.e. Coordinates/clarifies data collection issues and questions by consulting with the data analyst.f. Coordinates issues relevant to data collection and quality of documentation.g. Establishes work schedule whereby requests are processed on a timely basis.<ol start="2"><li>Performance Improvement/Education</li></ol>a. Attains accuracy standards within six months of employment and maintains thereafter.b. Demonstrates a productivity level that is consistent/supports personal and department goals.c. Participates in quality improvement activities and serves on Performance Improvement teams as appropriate.d. Completes annual departmental mandatory requirements i.e., Corporate Compliance Training.e. Provides feedback to department after attending internal/external in‐services.<ol start="3"><li>Performs clerical functions necessary to support department operations</li></ol>a. Uses appropriate telephone etiquette.b. Utilizes email/IDX, EPIC, PowerPoint, Excel, etc.c. Maintains department operational manuals. d.
