Our interdisciplinary team includes a Physician Medical Director, a Nurse Practitioner, Nurse Case Managers, Certified Nurses Aides, Social workers, Chaplains, Volunteers, a Volunteer and Bereavement Coordinator, Hospital pharmacists and Therapists, our amazing office support team, as well as support from ancillary departments through our organization who all strive to provide excellence in quality compassionate care with a high degree of professionalism, knowledge and skill. Someone with a passion for end of life care, who can critically think through variety of situations and who provide the highest compassion and caring support for our patients and their families, our volunteers and our community.
