No serious violations in the past three (3) years as listed on the MVR or PSR (Including driving under the influence, license suspension, felonies, possession of a controlled substance, reckless driving, hit and run accidents, eluding police, violating a state or local law prohibiting texting or use of hand-held mobile phones while driving a CMV. No preventable crashes involving the following situations in the past three (3) years as listed on the MVR, PSR or driver application (at fault rear-end collisions, rollovers or jackknife, head on collisions).
