p>Scope of Responsibilities: Work collaboratively with the primary care providers to identify at-risk patients in need of health education as well as broader population needs around health promotion and disease prevention, including diabetes education, weight management, cardiovascular disease, hypertension, renal disease, malnutrition / weight gain, infants with failure to thrive, cancer and nutrition therapy, gastrointestinal disorders, nutrition therapy, sports nutrition or healthy diet, and other chronic diseases. This position includes, but is not limited to: designing, promoting, implementing and evaluating comprehensive, high quality health education activities that focus on at-risk populations, in order to increase positive health outcomes.
