The External Marketer/Liaison will establish professional relationships with hospital discharge planners, social workers, case managers, physicians’ practices, and the general community to develop awareness of the healthcare services that JKV provides to the community, generate new referrals to support JKV's census and quality mix goals, and ensure a smooth transition for JKV’s current and future residents. Foster high-value relationships with hospitals, physicians, discharge planners, social workers, senior living communities, home health agencies, rehabilitation providers, elder law attorneys, financial planners, trust officers, insurance professionals, senior support organizations, and civic/community groups to enhance collaboration and referral growth.
