Working closely with clients, families, referral sources, medical professionals, and the interdisciplinary treatment team, the Assessment Counselor will help ensure each individual receives appropriate, coordinated, and clinically informed care. This role is responsible for evaluating presenting concerns, determining appropriate levels of care, identifying immediate safety needs, and supporting timely admission and referral decisions.
