Regularly present findings to: Hospital executives and C-suite leadership, Health system leadership, Payer and managed-care partners, Physician and primary care organizations, Skilled nursing and post-acute leadership, ACO and Population Health leadership, Internal executive leadership and Other strategic partners. Experience with hospital medicine, skilled nursing facilities, care transitions, Medicare and Medicare Advantage populations, ACOs, Population Health, Care Management, value-based care, payer relationships, or multi-state physician organizations is highly desirable.
