Associate's degree or above leading to a valid Tennessee Board of Health issued license in one of the following areas or occupations: registered nurse, physician, chiropractor, dentist, dental hygienist, medical laboratory technician, paramedic, athletic trainer, occupational therapist, physical therapist, respiratory therapist, radiology technician, diagnostic medical sonographer, nuclear medicine technologist, audiologist, veterinarian, physical therapy assistant, occupational therapy assistant, physician's assistant, speech therapist, optometrist, pharmacist, dietician, podiatrist, clinical perfusionist. Minimum of three (3) years of work experience completed within the last five (5) years, five (5)years of relevant work experience, completed within (10) years, in the area of endorsement or equivalent education attainment as defined by the department, documented in the Occupational Experience and Qualification Form .
