Demonstrates effective coordination of community employment services that adhere to person-centered planning through collaboration with individuals and/or parents/guardians, Case Managers, clinicians, and other community organizations. Ability to walk, bend, stand, sit, lift up to 25 pounds, with or without assistance, stretch/reach, hear, see, hand/finger dexterity, and drive.
