Your attention to detail and collaborative approach will help create personalized care experiences that truly make a difference in the lives of those we serve. </div><br/><h3>Key Responsibilities</h3><br/><div>Participant Needs Assessment and Care Planning</div><ul><li>Conduct initial and periodic assessments of participant needs in conjunction with the Interdisciplinary Team (IDT).</li><li>Recommend interventions to ensure participant function is maximized in the home setting.</li><li>Develop and coordinate an emergency plan of care in the home setting with the IDT.</li></ul><div>Home Care Coordination</div><ul><li>Arrange assistance for Activities of Daily Living (ADL) and Instrumental Activities of Daily Living (IADL) deficits in the home environment.</li><li>Arrange home respite care to alleviate caregiver stress.</li><li>Coordinate and provide personal care supplies and DME.</li><li>Provide medication reminders in the home setting as directed.</li></ul><div>Therapy & Equipment Coordination</div><ul><li>Work with Physical and Occupational Therapy to ensure participants and/or caregivers understand durable medical equipment.</li><li>Facilitate coordination of home exercise programs with therapy and home health aides.</li></ul><div>Home Visits & Monitoring</div><ul><li>Conduct regular home visits to ensure services align with the participant's functional goals and meet the needs of the caregiver.</li><li>Monitor the effectiveness and appropriateness of the care plan, making necessary revisions as conditions change. </li></ul><div>Communication & Collaboration</div><ul><li>Regularly communicate the appropriateness of the care plan to the IDT.</li><li>Facilitate bi-directional communication of the care plan to ensure the agency understands the plan and its requirements.</li><li>Monitor and ensure that the care plan is being followed.</li></ul><div>Technology Utilization & Coordination</div><ul><li>Effectively utilize technology and software to carry out duties, monitor care, and coordinate services.</li></ul><div><em>The above is a summary of the position, it in no way states or implies that these are the only duties this position will be required to perform. You will coordinate a variety of home care services—including personal care, homemaking, respite, and community living support as well as durable medical equipment and supplies.
