JOB TITLE: Home Health RN Case Manager & Recertification Nurse (RNCM)This is a Part-Time or PRN leadership position with our agency, with two employment options based on travel requirements:PRN for up to 1 hour (60min) travel, or PRN or Part-Time for up to 2-hour travel radius (and first priority of cases to secure additional hours) HIGHLIGHTS & REQUIREMENTS:This role works closely with and reports directly to our NW OH Regional Program Administrator. Work consists of: case management, starts of care (SOC), admissions, recertifications, transitional care and resumptions of care (ROC), clinical reviews, and other assessments and services, as well as new hire orientations, in-service and supervisory reviews, and input to influence our training and education, care practices and outcomes, and the success of our clients, employees and business overall.
