Foster Meaningful Connections: Build and nurture relationships with healthcare professionals, providers, community-based organizations, senior housing communities, post-acute facilities, and senior centers to encourage referrals to the myPlace Health PACE program across Greater Los Angeles . Community-Focused Role: This position is primarily field-based, working in vibrant community settings such as government offices, resource centers, senior centers, tiny home communities, and other areas ideal for generating referrals.
