Watch our "Now Hiring" TV commercial: Responsibilities * Provide skilled in-home physical therapy services * Complete evaluations, reassessments, and fall risk assessments (PT) * Implement individualized treatment plans to improve mobility, balance, strength, and safety * Educate patients and caregivers on home exercise programs and fall prevention * Document accurately and communicate patient progress with the interdisciplinary care team * Maintain compliance with Medicare home health documentation requirements Requirements * Current Hawaii license as a Physical Therapist (PT) or Physical Therapist Assistant (PTA) * Minimum one year of clinical experience (home health experience preferred) * Strong documentation, communication, and time-management skills * Ability to work independently in the field * Valid driver's license and reliable transportation * Passion for serving seniors, kupuna, and local families Why Join Our Team?
