ACLS credential (or must obtain ACLS within 6 months of hire).Licensed as a Respiratory Care Practitioner by the New Jersey State Board of Respiratory Care. Demonstrates competency in all aspects of the Blood Gas Laboratory and/or Laboratory procedures regarding Blood Gases (location dependent).
Uses own transportation to provide care at patient’s residence: Must have and maintain valid PA or NJ Driver’s License; Must have and maintain a good driving record accepted by the Network insurance carrier; Must have and maintain motor vehicle insurance with the minimum requirements established by PA or NJ; Must obey and adhere to all motor vehicle regulations and laws while operating vehicle; Must perform routine maintenance on vehicle. Maintains and completes evaluations, patient records, reports and all written documentation effectively and in a timely manner, in accordance with regulatory and department guidelines. • Consults with medical, ancillary professionals and other team members/supervisors to provide optimum quality patient care and effective operations.
Services include, but are not limited to: (a) Assisting with activities of daily living, (b) Budget planning and money management, (c) Maintenance of stable housing, (d) Accessing benefits and entitlements to increase financial independence, (e) Meal planning and grocery shopping, (f) Access to reliable transportation services, (g) Side-by-side coaching and training, (h) Transport and accompany clients to scheduled community activities or appointments. The Co-Occurring Specialist provides primary support to ACT participants and provides cross-training to the other members of the ACT team—to assist with substance abuse recovery and harm reduction—with the goal of promoting stability and increasing community tenure.
