The Care Manager will assist clients to access services, such as primary care, substance use treatment, mental health counseling, transportation, intensive care management, housing, and vocational/educational resources, through phone calls to clients and providers, and home visits when needed. The Care Manager will provide all aspects of care coordination for SAMHSA AIM 2.0 clients: complete initial enrollment (all required SAMHSA and program forms/assessments, including the SURPT Tool), develop care plans, promote compliance with appointments and treatment regimens, and encourage self-sufficiency.
