Participate in weekly meetings and monthly in - service trainings Education and Experience: Required: Bachelor's degree in health education, nutrition, exercise physiology, social work, psychology, human services, counseling, education, or related field Ability to write reports and correspondence Moderate level of sophistication using web apps, able to use information dense productivity tools Passion for helping others with empathy, listening and perception skills Excellent organizational, interpersonal, and communication skills Certified or willing to obtain certification in a nationally recognized coaching program Strong analytical and problem solving skills Strong relationship-building and problem-solving skills Preferred: Experience with the use of behavioral techniques such as goal-setting, problem-solving and/or motivational interviewing Experience in direct education and/or coaching with individuals for disease prevention, health promotion and behavior change Certified in a nationally recognized coaching program Care Coaches help members set goals, adopt self-management skills, as well as identify and overcome risks and barriers, ultimately supporting behavior change and leading to positive health outcomes.
