The Nurse Navigator serves as the day-to-day workflow lead for the Patient Navigators and collaborates with physicians, the APRN, Registered Nurses, CBOR staff, pharmacies, infusion services, and internal and external healthcare partners. Performs clinical chart preparation for new rheumatology patients by reviewing available referral documentation, outside medical records, laboratory results, imaging, prior specialty evaluations, medication history, and other relevant clinical information before the initial visit.
