In this role, you will evaluate patients for hospice eligibility, educate patients and families on hospice services and benefits, complete the admission process, and develop an individualized plan of care that ensures a seamless transition into hospice services. The Admissions RN partners closely with physicians, referral sources, clinical leadership, and the interdisciplinary hospice team to deliver a timely, compassionate, and coordinated admission experience.
