What you bring:** + Master's Degree Required: Social Work from an accredited graduate school of social work + 2 years Required: clinical experience with designated population and + Preferred: Hospital, emergency room, chemical dependency, or community agency experience and + Required: Clinical Social Worker Required Credentials LICSW (WA), SWAL (WA), SWAICL (WA), or SWAAL (WA) **Skills** + Familiarity with medical terminology and conditions as these impact discharge needs (Required) + Excellent verbal and written communication skills (Required) + Must possess superior ability to work collaboratively within a team (Required) + Must possess excellent organizational skills (Required) + Ability to document on electronic record (Required) + Knowledge of community resources, knowledge of Medicare, Medicaid and insurance requirements as these relate to continuing care (Preferred) **Department / Location Specific Notes** PHMG: + In lieu of MSW, a Masters degree in Counseling or related field with a minimum of two years work experience in a medical or healthcare setting, social service agency, or community organization focusing on health and/or welfare issues may be accepted.
