Duties:Build and maintain referral partnerships across healthcare and community networks Conduct outreach via in-person visits, calls, events, and ongoing follow-up Generate and manage a consistent pipeline of qualified opportunities Guide families through care options using a consultative approach Conduct client assessments and recommend appropriate solutions Manage the full lifecycle from initial inquiry through start of service Track pipeline activity, conversion metrics, and referral engagement Collaborate with internal teams to align growth with staffing capacity. Requirements:2+ years experience in sales, business development, account management, or relationship managementExperience in healthcare, senior services, in home health care, or referral-based industry preferred CRM experience and proficient in MS Office Suite Proven track record of meeting or exceeding performance goals.
