As a Home Care Registered Nurse (RN) with Mercy Home Health, youll bring high-quality, compassionate care directly to patients where they feel safest and most comfortable: their homes. p>Shift: Description: We are seeking a dedicated Home Care Supervisor to oversee Certified Home Health Aides (CHHAs) and Certified Nursing Assistants (CNAs) delivering care in home settings.
A Catholic-based, non-profit organization, we serve patients and their loved ones with home care (skilled nursing, physical/occupational and speech therapy and medical social work), palliative care, hospice and bereavement services. We have a pioneering care model with Home Care Connect, our integrated virtual care program that helps patients avoid preventable ER visits and hospitalizations.
You will develop treatment plans tailored to each patient’s unique needs, using a holistic approach to promote recovery and enhance daily living skills.
