Provide nursing care, promote wellness, and monitor health needs; Coordinate care with doctors, clinics, and hospitals; Support medical appointments and hospital transitions; Educate individuals and staff on health conditions and care plans; Maintain medical records, update EMAR, and ensure medication compliance; Train and support staff on medical needs, competencies, and care plans; Perform specialized care (G-tubes, J-tubes, trach care); Support medication ordering and inventory processes. Educate individuals and staff on medical conditions, treatments, and care plans (e.g., allergies, medications, diet); Provide guidance on chronic and acute health needs; Foster open, respectful communication with individuals served, families, and care teams .
