Job Summary: Qualified candidates must be able to perform the following job functions: Visit with and establish an effective rapport with physicians, discharge/admission coordinators, social workers, community liaisons and other health care providers in order to generate home health and hospice referrals. Coordinate business development and education of patients, insurance companies, physicians, skilled nursing facilities, assisted living facilities, rehab facilities, independent living facilities, nurses and other community health providers.
