At launch, this is a hands-on operational role with direct responsibility for intake: the Team Lead personally performs all intake activities for partnership referrals — working the referral intake queue daily, verifying that referral data is complete, performing insurance verification, assigning providers, and advancing patients through the intake pipeline — in addition to coordinating outbound referrals and serving as a primary point of contact for partner communication. Work the shared care coordination platform's intake queue daily: confirm all referral data is present and complete, perform insurance verification, create the patient chart in the EMR, assign a provider where needed, and advance each patient through pipeline stages with documented reasons for any patient who does not proceed .
