Communication - Communicates any unusual findings and/or occurrences, obvious changes in patient’s physical, mental or social status, home safety hazards, refusal of care, or patient/family dissatisfaction to the primary nurse and/or Home Health Aide Supervisor. The Mission of IJN Health Systems is to provide comprehensive, high-quality home care services to our patients by creating strong partnerships with their families, case managers, discharge planners, and physicians.
IJN Health will meet the needs of our communities and patients, and collaborate with the Post-Acute Care Facilities by providing high-quality, individualized, and comprehensive Home Health Care Services.
