The ENTRY PSR I/Lobby Experience Coordinator (LEC) helps with patient care by greeting patients upon arrival and answering any questions or concerns with care and compassion. The Registered Nurse coordinates total plan of care with other health care professionals involved in care and helps to achieve and maintain continuity of patient care by planning and exchanging information with physician, agency personnel, patient, family, and community resources. - Develops, implements, and updates patient plans of care in collaboration with physicians and the interdisciplinary team; ensures home‑health eligibility and completes comprehensive nursing assessments (including OASIS and medication reconciliation).
li>Quality- Makes initial and/or comprehensive nursing evaluation visits, ensures patients meet home health eligibility and medical necessity guidelines, determines primary focus of care, develops the plan of care within State guidelines with the physician, and submits accurate documentation.
- Actively participates in survey/survey readiness activities and performance improvement plans, works to reduce unnecessary patient hospitalizations, improve patient safety, and implements processes and best practices to ensure positive patient outcomes.
