As a myLaurel Clinical Liaison, a typical day will include attending daily hospital rounds and huddles, reviewing patient census, completing chart reviews, referral tracking, and providing education and engagement for hospital staff, providers, patients, and caregivers. As a tech-enabled medical group, we provide on-demand acute and transitional care to frail, elderly, or medically complex patients at home—helping them avoid the traditional pathway of ambulance, emergency department, hospital admission, and post-acute facility care.
