Other role expectations and requirements:Be trained and credentialed for all health plan clients and lines of business in primary coverage state(s)Be accountable to Signify Health operational, clinical, and member experience key performance indicators and metrics Conduct Video Visits as needed (maintain training, affiliation, and compliance with Video Visit requirements, regular training required)Maintain training and complete all diagnostic and preventive service (DPS) products as directed (regular training required)Maintain all required supplies and associated diagnostic equipment related to DPS offeringsMaintain active medical licenses (MD/DO) in coverage area(s) Maintain multi-state licenses (preferred)Experience with Signify Health In-Home Health Evaluations, in-home visits, or a similar format (preferred)Be flexible to travel locally and within licensed states, as needed (up to 25%)Willingness to execute a collaborating physician agreement, partnering with and overseeing Signify Health-contracted nurse practitioners or physician assistants and fulfilling all state-by-state requirements (preferred)Ability to sit, stand, bend, reach, push, pull and lift up to 20 pounds of supplies (including, but not limited to, iPad, medical supplies and scale), requiredAccommodate field observations and/or \u201cridealong\u201d experiences, as requestedDuring our In-Home Health Evaluations (and other associated follow-up visits), you\u2019ll meet people where they are and re-engage them with their health care during an approximately 45-minute pre-scheduled visit.
