Description: Job Summary: In general, the Palliative Care Nurse Coordinator is responsible for screening palliative care referrals/ evaluating the appropriateness of palliative care referrals, gathering patient and family data in preparation for the palliative care consult by the palliative care team, coordinating palliative care consults, assessing member palliative are needs and monitoring the quality of outcomes.
