li>Participates as part of the multi-disciplinary health care team to develop safe and timely coordination of care including but not limited to post-acute placement, palliative/hospice service lines, medical equipment, home healthcare, outpatient follow up, mental health resources, and other community resources. A licensed, masters-prepared social worker who is an experienced and independent generalist clinician responsible for complex psychosocial interventions and facilitating the plan of care through discharge planning and resource utilization for all patient populations throughout the Northwestern Memorial Hospital campus. Gynecological/obstetrical-related issues including teen pregnancy, parenting issues, adoption planning, infant developmental problems, drug exposed neonate, fetal death, unplanned pregnancy, pregnancy termination, and other care as needed.

As the Licensed Master Social Worker you are responsible for conducting psychosocial evaluations, developing knowledge, and understanding the needs for the clinically complex dialysis patients.
Responsibilities include, but are not limited to, assisting patients in navigating complex financial/insurance issues, providing supportive counseling to patients in their re-adjustment to pre and post-transplant lifestyle and management of complications, and making referrals to outside mental health providers and agencies as necessary. Additionally, the program has the largest bilingual and bicultural transplant team of more than 40 specially trained professionals dedicated to serving patients in need of a transplant based on their cultural and language preferences.
