In this role, you will provide psychosocial support to patients and their families while assisting with discharge planning, crisis intervention, and care coordination across a variety of medical and surgical settings, including transplant, pediatrics, and outpatient services. You will also connect patients with community resources, including home care, skilled nursing, and acute rehabilitation, and provide support during complex situations involving abuse or neglect, substance use, housing instability, and other barriers to care.
