Common diagnosis: LTC/SNF patient population: DM, COPD, Post Stroke, heart disease, fractures, dementia (Secure care tracker), Alzheimers, Rehab, chronic conditions, multiple comorbidities, and more. - Shifts & Scheduling: Days: 7 A-3 P or 7-7 (typically 3 RNs and/or LPNs, 4-5 LNAs on each floor & 1 MNA), Evenings: 3 P-11 P, Nights: 11 P-7 A, 7-7 (typically 2 RNs and/or LPNs, 4-5 LNAs on each floor).
