In this position, the intern will support the hospital's efforts to reduce length of stay and increase discharge home by gathering, analyzing, and reporting on key post-acute performance data, such as length of stay, readmissions, discharge to home rates, and discharge volume to quality post-acute care providers, like nursing homes and home care agencies. The Post Acute Care (PAC) Services team supports NYP's care teams by identifying and operationalizing resources to facilitate optimal discharges from the hospital.
