The NP oversees hospitalized participants in collaboration with a hospitalist including discharge planning makes home visits as needed, participates in weekly intake meetings, daily morning meetings, family meetings, and clinical case conferences, and supervises student interns and volunteers. Responsibilities include performing comprehensive intake assessments and quarterly reassessments, developing and monitoring individualized plans of care with an interdisciplinary team, evaluating and treating episodic illness, ordering and interpreting diagnostic tests, and performing procedures such as injections, immunizations, and EKGs.
