Interdisciplinary Team: PT/OT/SLP, Pulmonary or Cardiac Rehab, Case Managers, Social Workers, Dieticians?: PT, OT, SLP, Case Managers, Social Workers, Dieticians. Serves as a resource for others throughout the shift, responsible to monitor change of condition, residents issues, schedule outside services (such as lab, portable x-ray, etc.), contact MD/NP for resident needs/issues.
