Prepare patient by collecting and sharing data; blood pressure, pulse, respiratory rate, height/weight, Date of LMP, general history and chief complaint and identifies need for medication refills. The Physician Practice team works with community agencies and resources in order to promote, restore, and maintain optimal health for patients and families in the surrounding area.
