ID#3939A - Under supervision, the Case Manager/Mental Health Counselor will function as part of a mobile case management team to coordinate and provide mental health evaluation, intensive case management services including plan development, placement services, assistance in daily living, and individual and group or family counseling to CMI clients. Assess client’s level of psychosocial impairment, self-care potential, family and support network availability, independent living needs, financial status, educational, employment or training needs, and social/recreational needs.
