Key Responsibilities: Provide clear, thorough discharge education to patients and families, including medications, follow-up appointments, and care instructions Reinforce physician and RN discharge plans to ensure patient understanding and compliance Collaborate with nurses, providers, case management, and support staff to facilitate timely and safe discharges Answer patient questions and address concerns with patience and empathy Ensure all discharge documentation is complete, accurate, and compliant Assist with coordination of transportation and post-discharge needs as appropriate Maintain a welcoming, calm, and organized discharge environment Promote a culture of teamwork, respect, and excellent customer service Qualifications: Current Louisiana LPN license in good standing Previous acute care or med-surg experience preferred Strong communication and patient education skills Ability to multitask and manage patient flow efficiently Positive attitude and team-oriented mindset Must demonstrate professionalism, compassion, and the ability to work well with all team members What Were Looking For: We are seeking someone who leads with kindness, communicates effectively, and contributes to a positive team environment.
