Licensed Practical Nurse (LPN): Provides nursing care, monitors client conditions, administers medications and performs authorized nursing procedures, and reports changes to the supervising RN, physician, or other appropriate clinician as required by law and the plan of care. Nurses will work collaboratively with clients, families, caregivers, physicians, and the KAFF Homecare care coordination team to ensure that services are delivered according to each client's individualized plan of care.
