The RN provides pro-active disease management using evidence based practices; acts as a liaison between patient/family, medical staff and others involved in treatment (ie ER, Hospitals, nursing homes, home health care and the community at large) promoting patient/family-centered care based on jointly determined health care goals and priorities as defined in the individual care plan; and coordinates implementation of health care home improvements (patient registry, care planning, health outcome data management).
