p>PRIMARY PURPOSE: To provide comprehensive, patient-centered nursing care within FoundCares Federally Qualified Health Centers (FQHC), supporting primary care delivery, chronic disease management, preventive services, and care coordination for diverse and underserved populations, including patients with complex medical and social needs, under the supervision of a Registered Nurse (RN) or provider.
Ability to travel to other locations to attend meetings, workshops, and seminars, plus travel to other FoundCare departments and FoundCare conference rooms.
