Utilizing nursing knowledge and experience, this role serves as a resource for participants with more complex needs, facilitates communication among participants, families, caregivers, and healthcare partners, and helps identify and address service-related concerns to promote a positive participant experience and high-quality service delivery. Licensed Practical/Vocational Nurse (LPN/LVN) candidates: Working knowledge of clinical documentation, care coordination, and applicable state nursing regulations within the licensed practical nursing scope of practice.
