Utilizes technology to enhance quality patient/resident care, collaborates with inter-professional team members, effectively delegates to and supervises unlicensed personnel, assumes overall nursing accountability for a patient/resident assignment, and serves in leadership roles on department/organizational task forces and committees. The Licensed Practical Nurse (LPN) Clinical Care Leader (CCL), Long-Term Care (LTC) is responsible for utilizing the nursing process (assessment, diagnosis, outcomes/planning, implementation, and evaluation) to provide individualized care in the long-term care setting while under the supervision of a registered nurse, advanced practice provider, or physician.
