The Clinical Support Supervisor will provide case consultation, reinforce evidence-based chronic disease self-management approaches, support clinical escalation protocols, strengthen coordination between CHWs and participating FQHC clinical teams, and contribute to training and quality improvement. This role will help ensure that CHWs are well equipped to support patients living with diabetes, hypertension, obesity, COPD, asthma, cardiovascular risk, and other chronic conditions while remaining strictly within the appropriate non-clinical CHW scope of practice.
