Individual must be accountable for the scope of their work and able to work independently to manage their caseload as well as with a team of 12 Case Managers to ensure the entire hospital receives needed Case Management services. Experience working with patients and families that have acquired brain injuries or with Medically Complex Patient Populations that have chronic disabilities and multiple social and medical community resource needs.
