The social worker responds to trauma activations, medical alerts, codes and staff assists on inpatient units providing crisis intervention, family support, grief and bereavement support, collaborates with teams on child abuse and neglect cases, works to remove barriers to timely discharge including transportation, medication assistance and other social drivers of health. About the Role: Part- Time 30 hours weekly- Evening Social Worker II role fosters positive and professional interpersonal relationships with patients/families, physicians, hospital staff, community agency representatives and visitors, provides comprehensive psychosocial assessment for identified patients and based on findings, facilitates interventions.
